1)         Complete contractor application (front & back)

2)         $5,000 dollar bond*

3)         Copy of the owner/applicant driver’s license

4)         $125.00 dollar licensing fee

 

 

CERTIFIED BACKFLOW PREVENTER

1)         Complete contractor application (front & back)

2)         Copy of owner/applicant driver’s license

4)         Copy of state license

5)         Copy of tester’s backflow test gauge calibration  

             form.

6)         No fee required for licensing

 

 

ELECTRICIAN

(i.e. Electrical Contractor & Electrical Sign)
